Dear Editor, Pancreatic cancer is a leading cause of cancer-related death worldwide, with a 5-year survival rate of approximately 4 % following diagnosis.1 Due to the lack of typical clinical symptoms in the early stages, 80e85 % of patients are diagnosed with advanced, unresectable disease.2 Current treatment strategies for unresectable pancreatic cancer include radiotherapy, chemotherapy, and immunotherapy.3 Nowadays, intraoperative radiotherapy (IORT) has shown promising results in improving the prognosis of patients with unresectable pancreatic cancer by delivering a single high dose of ionizing radiation to the pancreatic bed.3 However, to date, no cases have been reported that combine IORT, chemotherapy, and immunotherapy for the treatment of unresectable, locally advanced pancreatic cancer.
