The cohort study conducted by Yu et al.,1 which utilized data from Norwegian and Swedish populations, captured our attention with its intriguing findings. The study revealed an eightfold increased risk of small bowel adenocarcinoma in patients diagnosed with Crohn’s disease (CD), a twofold increased risk of small bowel adenocarcinoma in patients diagnosed with ulcerative colitis (UC), and a twofold increased risk of neuroendocrine tumors in patients diagnosed with both CD and UC. However, because of the relatively low absolute excess risk associated with developing small bowel cancer, implementing early surveillance measures may not be cost-effective. The assessment of causality in observational studies is often challenging because of the presence of multiple confounding factors. The existence of a causal relationship between inflammatory bowel disease and small bowel malignancy remains unclear at present.
